Participant Accident (Excess) Coverage
This coverage is provided for registered members participating on all USA Hockey registered ice hockey teams and registered officials. Covered medical and dental expenses are payable, secondary to any valid and collectible insurance the registered member has in force at the time of the accident. The member must meet a $1,000 out of pocket expense obligation, per accident, before this coverage begins. If the registered member has no valid and collectible insurance in force at the time of the accident, a deductible of $3,500, per accident, will apply.
To file an accident claim, contact your local registrar for an accident (claim) form. Officials should contact the District Referee-In-Chief for the claim form.
Participant Accident (Excess) provides coverage, on an excess basis, for accidental medical expenses, accidental death and dismemberment, and/or paralysis resulting from an accidental bodily injury while participating as a member of a registered team during a USA Hockey sanctioned game, a USA Hockey sanctioned tournament game, controlled scrimmage or practice session involving ice hockey. The coverage also extends to volunteers of USA Hockey while acting within the scope of their direct responsibilities on behalf of USA Hockey. A member is also covered on an excess basis while traveling, as a team, directly to and from a sanctioned game or official tournament in which their team is scheduled to participate. Coverage to and or from practice sessions is excluded.

Non-Sanctioned USA Hockey Tournaments or Events
Note: McKinney Ice Hockey requires that all Club teams participate only in events that are properly sanctioned under USA Hockey or the applicable governing body. Participation in non‑sanctioned events can result in a complete loss of USA Hockey insurance benefits for players, families, coaches, and the Club.
Sanctioned event requirement
McKinney Ice Hockey teams, coaches, and volunteers may not participate in games, tournaments, showcases, camps, or other events that are not sanctioned by USA Hockey (or another governing body formally recognized by the Club’s Board), unless expressly approved in writing by the Board of Directors in advance. Any such approval must identify the event, dates, governing body, and any alternate insurance protections in place.
Insurance and disclosure to families
Parents and guardians register their players with USA Hockey with the expectation that the benefits of that membership, including insurance protections, apply to Club activities. The Club will not knowingly place families in situations where an injury occurring during a team activity is outside USA Hockey insurance coverage because the event is non‑sanctioned. If the Board ever authorizes participation in a non‑sanctioned event, the Club will provide written notice to all affected families in advance, stating clearly that USA Hockey insurance may not apply and describing any alternative coverage the Club or event operator has arranged.
Board review and legal/insurance consultation
Before authorizing participation in any non‑sanctioned event, the McKinney Ice Hockey Board of Directors will review the risks, including potential loss of USA Hockey insurance coverage and increased liability exposure for the Club and its volunteers. The Board should consult qualified legal counsel and/or the Club’s insurance professional as needed to evaluate potential liability and to determine whether additional or alternative insurance is required to protect the Club, its directors, officers, volunteers, players, and families.
SANCTIONED EVENTS
Requirements :
For an event to be sanctioned all players, coaches , and on ice officials, and teams must be properly registered with USA Hockey. All practices are sanctioned and all games and scrimmages are sanctioned provided both teams are properly registered. Invitational tournaments games are considered sanctioned events providing the tournament has received USA Hockey sanctioning. Other on-ice activities may be approved by the District Registrar or his/her designee as sanctioned events (for example, bingo, pancake breakfasts, etc. ). Contact your district risk manager or registrar for details.

Coverage Benefits for Eligible Members
Accidental (Excess) Medical Expense – The coverage may pay up to a maximum of $50,000 for covered medical expenses incurred for medically necessary treatment required as a result of an accidental bodily injury. There are limited dental and physical therapy benefits. The first bills for any covered expenses must be incurred within 30 days of the accidental bodily injury.
- $10,000 Accidental Death and Coma – Loss must occur within one year from the date of the accident
- $12,500 Loss of Sight
- $18,750 Dismemberment or Paralysis of an arm or leg
- $12,500 Dismemberment of hand or foot
Covered Expenses mean those expenses incurred within two years from the covered accident date and submitted no later than 15 months from when the expense incurred. Do Not Wait until your primary insurance has completely finished your claim before sending in your USAH claim form.
CATASTROPHIC COVERAGE
Catastrophic injury insurance provides $2,000,000 insurance for a seriously injured member’s medical, dental or rehabilitation expenses which are greater than $50,000, which is the policy deductible. An important policy benefit is the provision of case management/assistance services.
What is a Covered Event?
A competition, game or event which is sponsored or sanctioned by USA Hockey or its affiliated associations and includes pre-competition activities and practice sessions, which are authorized, organized and supervised by the team’s coach or other adult designated by the team’s coach to supervise and coordinate such practice activities. The competition and practice sessions will be comprised of players, coaches and officials that are registered with USA Hockey and the players and coaches will be on a registered team. Travel to and from practice sessions is excluded.
What is a Catastrophic Injury?
An injury that is sustained by an insured person during participation in a sanctioned event or covered activity, including:
- While participating in a sanctioned event or performing directly assigned duties in connection with a sanctioned event: or
- While traveling as a team to and from the location of a sanctioned game; or
- While the insured person is engaged in an activity or on travel that is authorized by USA Hockey’s National Staff, during a temporary stay at the location of a sanctioned event; and
- Which results in a bodily injury to the insured person, who incurs a minimum of $50,000 or more of medical or dental (or a combination of these) expenses, hereafter called the deductible) which is expended within two (2) years from the date of the covered accident
Note: make sure you are playing in a USA Hockey sanctioned event (tournaments) and playing USA Hockey registered teams with USA Hockey officials displaying the current crest on the sweater; crests expire on November 30 each year.
Losses of this nature should be immediately reported to the District Risk Manager
Participant Claim Filing Tips Under USA Hockey Excess Medical Plan
PARTICIPANT CLAIM FILING TIPS UNDER THE USA HOCKEY EXCESS MEDICAL PLAN:
If you are a registered USA Hockey member and you are injured while participating in a USA Hockey
sanctioned/approved activity, follow these procedures:
- At the hospital or doctor’s office, always provide them with your individual, employer health plan, union plan, etc., information as your primary plan.
- The USA Hockey plan requires that treatment following an injury must occur within 30 days from the injury date.
The Next Business Day
- To file an excess accident claim, go to usahockey.com/FilingAClaim. If you have no other valid and collectible coverage, then your USA Hockey plan will be the primary plan with a $3,500 deductible.
- DO NOT DELAY filing your USA Hockey claim. The policy has a timely filing provision and you do not want to jeopardize your claim by not filing within the required time. DO NOT WAIT UNTIL YOUR PRIMARY INSURANCE HAS COMPLETELY FINISHED YOUR CLAIM BEFORE FILING YOUR USA HOCKEY CLAIM.
- Make sure your claim is electronically approved by the appropriate designated person.
- ONLY CLAIMS ARISING FROM A USA HOCKEY-SANCTIONED/APPROVED ACTIVITY WILL BE HONORED.
- Upon receipt of your acknowledgment with your claim number from A-G you should provide the hospital or doctor’s office with your USA Hockey plan information as your secondary plan.
- By giving all of the medical providers both your primary and secondary information, they will automatically send A-G the proper itemized medical bill and your primary insurance Explanation of Benefits (EOB), thereby, removing this responsibility from you of collecting the proper information to send with your USA Hockey claim. By following this instruction, your A-G claim administrator will be able to contact the medical providers to request information without being told they cannot release anything because A-G is not on file with them.
- If submitting as a secondary claim, each itemized bill must be accompanied by your primary insurance Explanation of Benefits (EOB), whether this is provided by the medical provider or by yourself.
- If you have paid any of your medical expenses, please indicate that clearly on the medical bills so that payment will be sent to you.

D&O Liability Insurance
USA Hockey members have coverage to protect themselves and their associations, clubs or teams from the threat of a lawsuit.
Coverage Includes:
- Liability limit of $1,000,000
- Deductible of $5,000 ($15,000 for non-profit Juniors)
- Acts beyond granted authority
- Wrongful termination, rejection or suspension of players or coaches
- Failure to deliver services
- Discrimination (based on age, race, sex, etc.)
- Lack of supervision/improper supervision
- Libel, slander and defamation of character
Directors & Officers coverage is provided for all youth and adult associations, non-profit Juniors and auxiliary organizations whose members are 100% USA Hockey registered.
Crime Coverage
Member organizations also have a policy to protect themselves against monetary loss caused by an employee or volunteer who might steal funds (theft, embezzlement, etc.).
Coverage for Affiliates and Districts
- Limit of Liability $100,000; Deductible of $10,000
Coverage for Associations, Teams, Clubs and Leagues:
- Limit of liability $50,000; deductible of $1,000
Note: USA Hockey’s crime coverage may not be adequate for all member organizations. Some organizations may control funds in excess of the maximum limits above.
